Each playslip has 10 play areas (A-J) with boxes numbered 1 to 39. Fill in the numbered boxes corresponding to the numbers you have chosen to play or select the Quik Pik ("QP") option. Select the eZmatch option, for an additional $1 per play, for your chance to win cash instantly! Check your playslip carefully, as a FANTASY 5 ticket cannot be canceled. Then present your playslip, along with $1 for each play you've marked times the number of drawings played, to the Lottery retailer to receive your ticket. Selecting eZmatch will add an additional $1 per play. Sign the back of the ticket so no one but you can claim the prize amount. You are responsible for the accuracy of your ticket.

Features

 eZmatch
You can choose to play eZmatch (for an additional $1 per play) by marking the eZmatch box(es) beside each play area or simply selecting eZmatch on all plays. Match your Fantasy 5 numbers to any of the eZmatch numbers and INSTANTLY win the corresponding cash prize! You can win eZmatch up to 5 times on each ticket.

Each Fantasy 5 with eZmatch ticket will print on a separate ticket. All eZmatch prizes can be cashed instantly at a Lottery retail location. The retailer will redeem the eZmatch prize on your ticket and give you an exchange ticket for the FANTASY 5 evening drawing, if redeemed prior to the drawing.

 Multi-Draw
You can play the same numbers up to 14 consecutive FANTASY 5 draws ($1 per play per draw) by selecting the Multi-Draw box on your playslip corresponding to the number of drawings you wish to play. The play must include the current draw and not skip any draw in between.

 Quik Pik
You can have the computer select your FANTASY 5 numbers by marking the box next to "QP" for "Quik Pik" on your playslip. Quik Pik gives you the option to have the computer randomly select some or all of your numbers.

FANTASY 5 cash prize amounts for matching 3, 4 or 5 numbers are pari-mutuel, all winners share an equal portion of the winnings in each prize category.

All prizes up to and including $600 may be claimed at any Lottery Retailer, Lottery District Office, or by mail. To claim by mail, simply complete the information on the back of the tickets and mail to:

All prizes may be claimed at any Lottery District Office or by mailing the prize to the above address. Winners of prizes over $600 must complete a Winner Claim Form and provide identification. Winner Claim Forms can be obtained at any Lottery Retailer or Lottery District Office. The preferred forms of identification are a valid driver's license and a Social Security card. If a prize over $600 is claimed by mail, photocopies of the required identification must be mailed along with a completed and signed Winner Claim Form.

Winning tickets must be claimed within 180 days after the draw date.
